I have booked an order for spectacles in your opical wing at Guntur on 22/11/2011,vide receipt No. SALORD-T99/0031 d/22-1-2011, after a test in your hospital and on prescription by Dr. Bharat Kumar on 22-11-2011.The spectacles are expected to be delivered to me by 27/11/2011. as per the receipt. -Complaining of bilateral corneal dystrophy.
-Keratoplasty was done in right eye in 2007, graft rejection in right eye, patient is in need for regrafting in right eye and penetrating keratoplasty.
